I am fully aware and understand that my examination and interviews may be videotaped. I understand that the polygraph examination to be administered to be shall consist of three parts.
I understand the first part will be an interview between the designated polygraph examiner and me. During this interview, the polygraph examiner will discuss with me all areas and subject matter upon which I will be tested during the polygraph examination. I further understand the designated examiner shall explain to me how I can cooperate in the examination in order to shorten the time it will take to conduct the examination and confirm the results of the examination.
I understand that the second part of my examination shall consist of the actual test. I understand that I shall be tested only on the areas and subject matter discussed in the interview that precedes the actual testing. I understand that I will not be tested nor asked any questions over any matters or subjects not discussed in my initial interview.
I further understand that the final part of my examination will be the delivery of all information gathered from the interview and polygraph examination, including the professional opinion of Forensic Technologies, Inc./Mr. Bullens as to the truthfulness of my answers, to my designated representative(s) to receive this information and none other. I further understand that once Forensic Technologies, Inc./Mr. Bullens delivers this information to my designated representative that they shall not, and will not, divulge this information to any other person, firm, partnership, corporation or other legal entity, unless compelled to do so through the legal process.
